To replace
a lost or stolen passport, applicant must meet the qualifications:
Applicant is a US citizen over 18 years old AND;
Applicant is unable to locate his or her passport OR;
Applicant's passport was stolen.
Note: If applicant has a valid or recently expired
passport in their possession, apply for a passport
renewal. If applicant has never has a passport, apply
for a new passport.
Replacing Lost (or Stolen) US Passport Process Overview
Applicant must gather documents listed below to take to an
Acceptance Agent.
An Acceptance Agent is a government employee who will
verify the identity of the applicant.
Acceptance agents are at designated Post Offices and
Court Houses. Click

Applicant must indicate that he/she is using a hand carry
service. Acceptance Agent will have the applicant sign the
application, seal documents in an envelope and return the
sealed envelope.
Sealed envelope is forwarded to It's Easy Passport for
processing.

1 - The Following Items Must Be Presented By
the Applicant to the Acceptance Agent
1.1)
Applicant must bring an original or
certified copy of a birth certificate, naturalization
papers or old passport.

1.2)
Applicant must bring a driver's license
or state-issued identification issued more than six
months ago. Official government or military picture
identification, are also acceptable.
Applicant must submit two (2) identical
passport photographs. These may be obtained at many
Post Offices, Kinkos,
Mailboxes
Etc or similar locations. Poor photos are the most
common reason for delay in passport issuance.
Applicant must pay government fees at
acceptance agent via check, cash, money order or debit
card (credit cards are not accepted).
There will be two fees; one payable to U.S. Department
of State for $135 and a second for $25 payable to the
acceptance agent facility. The Acceptance Agent will
instruct the applicant regarding these payments.
1.7)
As part of the replacement process, applicant
must explain the circumstances regarding the recent
loss or theft using government form DS-0064.
On the application, applicant will be asked questions
regarding the loss of previous passport. Answer the
questions clearly and concisely as if speaking directly
to a US State Dept. official. Questions regarding your
previous passport number, expiration date, etc may be
left blank if information is not available
Applicant must submit one completed new
passport application form (DS-11). Application
must be filled out via US State Dept web site link below.
This information is not submitted to the government,
it is used to create a bar coded application required
to expedite the application; hand filled applications
are not acceptable for expedited services.
After filling out application
online, print two (2) completed applications
as instructed. One application will be sealed in the
envelope by the Acceptance Agent. It is important
that It's Easy be provided the second copy for reference
in order to process your application.
DO NOTsign the application
until requested to do so onsite by the Acceptance Agent.
