Name Change Overview (Detailed Instructions Below)

Changing the name on your valid US passport is easy. Name Changes are common due to marriage, divorce and other legal reasons. Applicant will send in original name change documents to have the name changed on the passport.

To perform Name Change quickly, applicant must meet the following qualifications:

Applicant has US passport is in his or her possession, AND;
Applicant has legally changed their name after most recent passport was
issued AND;
Applicant's passport was issued within the past 15 years AND;
Applicant applicant was 16 years or older when passport was issued.

If applicant does not meet the above criteria, please apply for a New US Passport.

Checklist of Requirements and Instructions for Passport Name Change

PART 1 - The Following Items Must Be Delivered to It's Easy

	1.1)

		Applicant must submit their current passport. Passport will be returned marked "Cancelled" by the US State Department.

	1.2)
		Applicant must submit original or certified copy of the marriage certificate (not a license) or a court decree ordering a name change or an adoption. The document must show the official seal of the issuing authority. All official documents will be returned.


	1.3)

		Applicant must submit two (2) identical passport photographs. These may be obtained at many Post Offices, Kinkos, Mailboxes Etc or similar locations. Poor photos are the most common reason for delay in passport issuance. Click here to view passport photo requirements NOTE: If applicant is an infant, ensure no device or hands used to hold child's head show in the passport photo. *NOTE:* If applicant is delivering documents to our offices in New York or Boston personally, passport photos can be taken on-site.

	1.4)

		Applicant must submit a computer-generated itinerary from a travel agency or online booking agency showing departure from the U.S. *NOTE:* If applicant does not posses proper itinerary, please contact us for options.

	1.5)

		Applicant must submit "Letters of Authorization" which allow It's Easy Passport and it's sister company to submit the application to the US State Dept. Click here to print the Letters of Authorization *NOTE:* All letters of authorization must be completed to ensure fastest processing

	1.6)

		The US State Dept charges $135 for expedited passport renewals. This fee may be made by personal (no starter checks), business check or money order made payable to the US State Dept. or may be combined with your service fee credit card payment to It's Easy when filling out the order form below. *Note:* If paying by check, inclide applicants date of birth on memo line.

	1.7)

		Applicant must submit one completed passport renewal application form (DS-82). Application must be filled out via US State Dept web site link below. This information is not submitted to the government, it is used to create a bar coded application required to expedite the application; hand filled applications are not acceptable for expedited services. After filling out application online, print completed application as instructed. DO NOT forget to sign and date the completed application once it is printed. 	1.8)
		Applicant must submit an It's Easy order form which provides contact information, service request and payment information. Click here to complete the Order Form

PART 2 - Ship Documents to It's Easy

2.1)

Ship all documents to It's Easy Passport. It's Easy recommends Federal Express for document shipping.
